What Insurance Paperwork Do I Need to Prepare Before Intake?

All we need is your first and last name, Date of Birth, a front and back copy of your insurance card. If your insurance card is not available, we need the name of the insurance company and member ID to verify benefits details.

In-network vs Out-of-network Rehab

It will depend on the details of the person’s policy. The best way we can help you answer this question is to check your insurance benefits to see what coverage you have available for substance abuse treatment.

Can My Insurance Cover Someone Else’s Rehab Expenses?

If they are a dependent or a member of your policy, then your insurance can potentially cover their treatment. If they are not a member or dependent you may be able to reach out to your employer and have them added.

Will My PPO or HMO Cover Rehab?

The primary differences between the two plans are monthly cost, the size of the plan’s network, and coverage for out of network.

Paying for Rehab with an HMO
Health Maintenance Organization: HMO plans tend to be me more affordable on a month to month basis but have a smaller network of providers for the member to choose from. HMO plans typically do not have out of network benefits and regularly require a referral.

Paying for Rehab with a PPO
Preferred Provider Organization: PPO plans tend to have a slightly higher month to month cost versus HMO policies. They offer a larger network of providers to choose from and do not typically require a referral from the member’s primary care physician.

Do You Accept Medicare or Medicaid?

We are not contracted with Medicaid or Medicare or any supplemental Medicaid or Medicare plans, however, we are happy to help guide you to find the right treatment center.
